The purpose of this extra credit is for you to log on to the survey website and complete the course surveys.  Go to www.cs.montana.edu/survey and log in with your LAST NAME and banner id.  You will see the following page with your name in the left corner:

Take a screen shot of the page (Alt + PrintScreen). Then complete the surveys. You should fill out the Faculty Survey (about me), the Course Survey (about the course content) and a TA survey (about your TA).  If I was you TA you can just add that information in the Faculty Survey.

All information is confidential.  Your student id is not kept with the information you provide.  All information you enter is used to make the course better for future students.

To get credit:  paste the screen shot into a word document and title it yourlastname_eval.doc.  Send this to your assignment mail box NO LATER THAN Tuesday May 3rd at 5:00. 

The survey will be online after that, but we need to receive it by that time to add it into your grade. 

If you don’t want to take the survey but still get extra credit you can click on each of the surveys and choose “decline”.  Then send the screenshot.

DO NOT SEND A SCREENSHOT OF YOUR SURVEY ANSWERS !!!

Send a screenshot of the example, (but it should have your last name on it).  The surveys are confidential.  I just want everyone to take them.
